Carelink of Georgia Linkage to Treatment Grant Application

Carelink of Georgia’s Linkage to Treatment Grant aims to support uninsured and underinsured individuals in the State of Georgia struggling with Opioid Use Disorder (OUD). The grant ensures comprehensive access to Medication for Opioid Use Disorder (MOUD) services through certified Opioid Treatment Programs (OTPs). These services include:

Required Services

  1. Physician Assessment
  2. Nursing Assessment
  3. Medication Administration
  4. Opioid Maintenance
  5. Diagnostic Assessment
  6. Individual Counseling
  7. Group Outpatient Services, including psycho-educational groups focusing on relapse prevention and recovery.
  8. Family Outpatient Services
  9. Addictive Disease Support Services
  10. Behavioral Health Assessment & Service Planning Development

Additional Services Provided

  • Crisis Intervention
  • Peer Support

Mandatory Requirements

To qualify for the grant, programs must meet the following mandatory requirements:

Licensing

  • Program must be in good standing with:
    • Georgia Department of Community Health (DCH) Narcotic Treatment Program
    • Drug Enforcement Administration (DEA)
    • Georgia Board of Pharmacy (GBOP)
    • Substance Abuse and Mental Health Services Administration (SAMHSA)

Accreditation

  • Accreditation from the Georgia Secretary of State (Business License).
  • Membership with Opioid Treatment Providers of Georgia (OTPG):
    • Non-members as of November 1, 2023, must wait at least one year to participate.
    • OTPG will verify membership status.

Operational Requirements

  • A valid lease agreement to ensure operational status for the grant program’s duration.
  • An attestation of no pending litigation.
  • Must not be the recipient of other DBHDD grant proceeds for MOUD treatment services.
  • Must collect and share treatment data for patient recipients.

Clinical Direction

  • The program must operate under the clinical direction of one of the following licensed/certified practitioners:
    • Certified Opioid Use Disorder Counselor (COUDC)
    • Certified Opioid Use Disorder Practitioner (COUDP)
    • Master Addiction Counselor (MAC)
    • Certified Advanced Alcohol and Drug Counselor (CAADC)
    • Certified Addiction Counselor II (CAC-II)
    • Georgia Certified Addiction Counselor II or III (GCADC-II or GCADC-III)
    • Licensed Professional Counselor (LPC)
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Marriage and Family Therapist (LMFT)

Staffing Requirements

  • At least one independently licensed/certified practitioner (as listed above) must be on-site during service operations.
  • At least one staff member must complete or register for all MOUD/WDT training series modules or hold a Practitioners for the Recovery of Opioid Use Disorder (PROUD) certification.
  • Certificates for each training module must be on file.
